“On November 11, 2016, I had a surgery performed at the Presence Saint Joseph Hospital located 2900 North Lake Shore Drive, Chicago, IL 606657. During that surgery, two X-rays were done and read by Doctors Arthur Heat and Lev Rapaport who work for Midwest Imaging Professionals LLC. On November 11, 2016, I had a surgery performed at the Presence Saint Joseph Hospital located 2900 North Lake Shore Drive, Chicago, IL 606657. During that surgery, two X-rays readings were done by Doctors Arthur Heat and Lev Rapaport who work for Midwest Imaging Professionals LLC. The Presence Saint Joseph Hospital and my Health insurance HealthScope Benefits have negotiated an IN-NETWORK contract (See attached document showing that Hospital is in my metwork). Since Midwest Imaging Professionals LLC is a company officially contracted by the Presence Saint Joseph Hospital it falls under the terms of that contract as an IN-NETWORK provider. Therefore, the claim from Midwest Imaging Professionals LLC has been processed by my health insurance according to terms of the contract they have with the Presence Saint Joseph Hospital. The Explanation Of Benefits I have received from my health insurance clearly indicates that the $104 billed have been excluded, and that there is NO patient liability (See attached document). Unfortunately, and despite multiples phone calls and letters, Midwest Imaging Professionals LLC has been harassing me to try to ILLEGALLY recover that excluded charge I am NOT liable for. They now have hired a collection Agency called Merchants' Credit Guide (888-249-3811) who sent me a letter on 12/11/2017 (see attached document) and is trying to collect those $104. I sent a letter dated 12/18/2017 to Merchant's Credit Guide explaining the situation, and providing a copy of my Explanation Of Benefits proving that I am not liable for the debt they are trying to recover. Nevertheless, they are since harassing me on the phone calling me every day!!",”
